How do I enroll?

  1. Check to see if you are eligible
  2. Complete enrollment forms
  3. Sign and return the enrollment forms
  4. Contact a patient educator in your area to make an appointment
  5. Bring your wallet card to your health screening appointment. Health screenings include:
    • In-office breast exam
    • Screening mammogram
    • Pelvic exam
    • Pap test
    • Blood pressure
    • Total cholesterol*
    • Body Mass Index (BMI)*
    • Blood sugar test*
    • *only for members 30+
  6. Receive results of your health screenings. If you need follow-up or treatment, WISEWOMAN and BCHP will help
  7. Take advantage of fun options:
    • Lifestyle counseling
    • Physical activity and nutrition assessment
    • Stress relief
    • Quitting tobacco
    • Goal setting
    • Group activities
  8. One year later, get screened again
