Reporting Patient Concerns

Please let your doctor or nurse know if you have any concerns while visiting a SEARHC location. To provide you with the best quality health care, we need to hear your concerns and ideas about how we may better serve you.

If you would like to further communicate any concern, please complete a Customer Experience Feedback Form (PDF 138kb) or write or call us about your concern. Our patient advocates can assist you.

The patient advocates are customer service liaisons for SEARHC patients and their medical providers. They listen, track and monitor trends based on customer feedback and recommend changes to improve SEARHC's performance.
