Download Foundation grants application
The Healing Hand Foundation application form is an Adobe Acrobat PDF file.
NOTE: Some browsers will only display a copy of the file and not download it to your computer. In this case, you can save the displayed file to your hard drive by clicking on the floppy disk (Save) icon in the top left corner of the file window.
- SEARHC patients: View and/or download the Healing Hand Foundation grants application form.
- Agency and clinic staff: Download the form you need to assist a SEARHC patient in applying. You will also need to download the main application form above and have the patient complete it.
