Applying for grants
1) Request our one page application form
You must complete the application in full, including back up information to justify the request.
- You can download an application form by clicking the link on the right.
- You can pick up a form at any SEARHC facility.
- You can also have a copy mailed to you by calling Mt. Edgecumbe Hospital Social Services (907) 966-8413.
- Agency and clinic staff assisting an applicant can get the form needed for this by clicking the download button on the right.
2) Get help from SEARHC staff
SEARHC staff can help with any questions about eligibilty or filling in the form. Click here for contact numbers.
3) Mail or fax the completed application
Mail or fax the completed application to Mt. Edgececumbe Hospital Social Services.
